  • Standard application
  • An electronic copy of the certificate of conformity or written notice to the manufacturer (manufacturer) that the ozone-depleting substances produced by him and (or) products containing ozone-depleting substances meet the requirements of the documents according to which they are made
  • Electronic copy of a valid insurance policy for cargo insurance or another document stipulated by the legislation of a Member State of the Union, confirming that the claimant importing ozone-depleting substances and / or products containing ozone-depleting substances is guaranteed in case of damage to human health and the environment
  • In the case of import of recycled and (or) recycled ozone-depleting substances, an electronic copy of the contract (contract) with the organization on the implementation of the recovery of ozone-depleting substances (submitted if the recovery is not carried out by the applicant) and confirming that the organization that plans to carry out the recovery of ozone-depleting substances corresponding to the established requirements
  • In the case of import of recycled and (or) recycled ozone-depleting substances electronic copy of the contract (contract) with the organization on the implementation of the destruction of ozone-depleting substances (submitted if the destruction will not be carried out by the applicant) and confirmation of the organization that plans to carry out the destruction of ozone-depleting substances for destruction corresponding to technologies for destruction of ozone-depleting substances approved by the Decisions of the Parties to the Montreal Protocol
  • In the case of import of ozone-depleting substances for use as raw materials, an electronic copy of the applicant's letter confirming the use of ozone-depleting substances solely as a raw material for the production of ozone-safe chemicals, or a copy of the agreement (contract) with an organization that will use ozone-depleting substances exclusively as a raw material for the production of ozone-safe chemical substances
  • An electronic copy of information confirming that the movement of ozone-depleting substances is carried out in a reusable container, if the legislation of a Member State of the Union prohibits the import (export) of ozone-depleting substances in a disposable container
  • An electronic copy of the contract (contract) on the provision of mediation services (if the intermediary acts as an applicant)

For individuals
